New clients will need to fill out several forms before therapy can begin. To make the process as smooth and efficient as possible, I utilize a HIPPA compliant online software for this. In order to schedule your appointment, I will send you an email with a link to the intake forms and scheduling portal. Please complete these forms prior to your first appointment. All forms are also available at the office if you do not have internet access.
Effective May 2018, my practice transition from an in-network provider and to a Direct-Pay Healthcare Provider. Payments may be made by cash, check, credit-card, or HSA card. Options for payment of services are:
Individual Session Fees – pay per session
- Initial Assessment $125
- Psychotherapy Session $125
- Couples/Family Counseling Session $125
- Group Sessions $25
Package plans – pay monthly
- Weekly Group – 4 sessions $80
- One session per month plus Weekly Group (4) $160
- Two sessions per month plus Weekly Group (4) $260
- Bi-weekly sessions – 2 per month $200
- Weekly sessions – 4 per month $400
- Four Weekly sessions plus Weekly Group (4) $460
